TRICARE Select

TRICARE Select is a self-managed, preferred provider organization plan available in the U.S. 

Step 1: Enroll in TRICARE Select

  • Enroll all family members on one enrollment form.
  • Send enrollment fees (if applicable) with your enrollment form.
  • These enrollment options are for new enrollments, TRICARE Open Season enrollments, and qualifying life event enrollments.

Mail or Fax

Mail your enrollment form to your regional contractor. The address is on the form.

Step 2: Get started with TRICARE Select

Your regional contractor will notify you when your enrollment is processed. Once enrolled, you can:

Were You Involuntarily Disenrolled?
If you would like to request reconsideration of involuntary disenrollment, late initial enrollment, or late change to coverage, contact your TRICARE contractor. 
Downloading TRICARE Forms

To download an enrollment form, right-click and choose “Save Link As” or download directly from the WHS forms page. For enrollment in TRICARE Prime options, TRICARE Select options, or TRICARE Young Adult, use your region-specific DD enrollment form.
