Purchasing Continued Health Care Benefit Program Coverage
To purchase CHCBP coverage, you must do so within 60 days of loss of TRICARE eligibility (regular TRICARE or through the Transitional Assistance Management Program). CHCBP coverage begins on the first day after you lose TRICARE eligibility.
To purchase coverage:
- Download and complete the Continued Health Care Benefit Program Application (DD Form 2837).
- Mail your completed application to:

You must send proof of eligibility and your initial quarterly premium payment with your enrollment form. Unremarried former spouses must also send a copy of the final decree of divorce, dissolution, or annulment.
Types of Plans
The CHCBP offers individual and family plans. Mark the type of plan you want when you fill out your enrollment application.

Changing Plan Types
The sponsor’s plan type can be changed from individual to family only if:
- The sponsor gets married.
- The sponsor has a baby or legally adopts a child.
- A child is placed as a legal ward in the home of a sponsor.
Sponsors can add a family member within 60 days of the qualifying life event. Their coverage is retroactive to the day of the event.
Newborn Enrollment
Did you or your sponsor separate from active duty service? If so, you no longer have a military affiliation. If your child was born or adopted after military separation:
- They can’t be added to the Defense Enrollment Eligibility Reporting System(DEERS) A database of information on uniformed services members (sponsors), U.S.-sponsored foreign military, DoD and uniformed services civilians, other personnel as directed by the DoD, and their family members. You need to register in DEERS to get TRICARE..
- You can enroll the newborn or adopted child in the CHCBP. Request a coverage card for your child.
Important: Have your provider attach a copy of the CHCBP card to all claim submissions. This will help ensure claims are processed properly.
Renewing Coverage
- You purchase CHCBP coverage in 90-day increments. You get billed quarterly for your premiums.
- Humana Military will send you a renewal notice 30 days before your coverage expiration. You must pay your renewal premium before the coverage expiration date printed on this notice.
- If your payment isn’t postmarked within 30 days of the coverage expiration date:
- CHCBP coverage will end.
- You won’t be able to repurchase the plan.
