Continued Health Care Benefit Program costs depend on your sponsorThe term sponsor specifically refers to the active duty, retired or deceased service member whose military status entitles their dependents (spouse, children, etc.) to receive TRICARE health benefits.’s military status before they separated:

  • Active duty
  • National Guard or Reserve
  • Retired

Premiums

You’ll pay a quarterly premium. 

  • Individual premium: $2,103 per quarter
  • Family premium: $5,339 per quarter

Outpatient Deductibles

Sponsor’s Former Status Deductible Amount

Active duty

Sponsor ranks E-4 and below: $66 per individual, but no more than $132 per family

Sponsor ranks E-5 and above: $198 per individual, but no more than $397 per family

National Guard or Reserve

Sponsor ranks E-4 and below: $66 per individual, but no more than $132 per family

Sponsor ranks E-5 and above: $198 per individual, but no more than $397 per family

Retired

Network deductible: $198 per individual, but no more than $397 per family

Non-network deductible: $397 per individual, but no more than $794 per family

Copayments and Cost-Shares

After you meet your deductible, you’ll pay a CopaymentA fixed dollar amount you may pay for a covered health care service or drug. or cost-share based on the type of care you get and type of provider you see (network or non-network).

Your CHCBP copayments and cost-shares follow TRICARE Select Group B costs for your beneficiary category before your sponsor separated:

  • Active duty family members (includes separated active duty service members)
  • National Guard and Reserve members and their family members
  • Retired service members, their family members, and other beneficiaries
