Case Management

We offer case management services for eligible beneficiaries with high-risk health needs. These services support people of all ages, from children to adults. Our case managers are here to guide you through each step of the process and assist you with accessing services in the TRICARE East Region.

Goals of Case Management

Help you understand your diagnosis and treatment options
Connect you with providers, equipment, and support services
Help plan a smooth transition when you leave the hospital or a care facility
Work to get your referrals and authorizations processed quickly
Coordinate with your doctors about your prescriptions
Help you understand how and when to take your prescriptions
Support you through changes in care, like moving to a new facility or stepping down from inpatient care
