My Military Health Records
Keep copies of your medical documentation, records, and healthcare history information. These can be helpful for your own reference.
- Do you get most care at military hospitals or clinics? You’ll have a DOD electronic health record in MHS GENESIS.
- Did you retire or separate after Jan. 1, 2014? Paper copies of your Service Treatment Records were digitized after your retirement or separation. If your STRs don’t contain sensitive information, they may be available on the MHS GENESIS Patient Portal.
- Do you want to request a complete copy of your health record? Complete a Request Pertaining to Military Records (SF 180). Submit this form to the organization listed on the form. You can also make requests in person at your last military hospital or clinic.
- Civilian providers may keep paper records. They also may use a private EHR system.

You can view your personal health information from MHS GENESIS. You can also download it. You can do this if:
- You get care at military hospitals and clinics.
- You’re a registered user on the MHS GENESIS Patient Portal.

Register for the Individual Longitudinal Exposure Record
Did you know you can register for the Individual Longitudinal Exposure Record? ILER is a secure, web-based application. It contains data about known occupational and environmental health exposures throughout your career. It also links known exposure events to incidents. ILER helps your providers individualize your healthcare.
