TRICARE Reserve Select & TRICARE Retired Reserve

To purchase TRICARE Reserve Select or TRICARE Retired Reserve coverage:

1. Log in to the milConnect.
2. Select “Benefits,” then “Beneficiary Web Enrollment.”
3. Follow the instructions to enroll or download an enrollment form.
4. Print and sign the completed Reserve Component Health Coverage Request Form (DD Form 2896-1). This form is only available through milConnect.

If you don’t qualify for TRICARE Reserve Select or TRICARE Retired Reserve, you won’t be able to complete or print the form. Contact your National Guard or Reserve personnel office for assistance.
