TRICARE Prime
Use the TRICARE Prime Enrollment, Disenrollment and Primary Care Manager (PCM) Change Form (DD Form 2876) to enroll in one of the TRICARE Prime Options:
- TRICARE Prime
- TRICARE Prime Remote
- TRICARE Prime Overseas
- TRICARE Prime Remote Overseas
To enroll, download, complete and mail your region's form to your regional contractor:

Enroll Online
- Go to the Beneficiary Web Enrollment portal
- Click on the red "Log On" link at the top of the page
- You must have a Common Access Card (CAC), DFAS (MyPay) Account, or a DoD Self-Service Logon (DS Logon) account to log in.
- Once logged in, select the “Medical” tab to enroll

- Overseas: Call the Regional Call Center for your overseas area
Downloading TRICARE Forms
To download an enrollment form, right-click and select to "save-as" or download direct from the WHS Forms Page. For enrollment, use your region-specific DD-3043 form.
