How to Submit a West Region Claim

In most cases, your provider will file medical claims after caring for you. Later, you’ll receive an Explanation of Benefits showing the services that are paid.

Sometimes you’ll need to file the claim on your own. Visit the Patient Resources page to learn more about when to file on your own.

Follow the steps below to file your own claim for care on or after Jan. 1, 2025.

• Go to www.tricare.mil/PatientResources/Claims/MedicalClaims.
• Download the DD2642 claim form and complete it.
• Submit the form in one of two ways.
  • Fax: 877-989-0070
  • Mail: TRICARE West Region Claims, P.O. Box 202160, Florence, SC 29502-2160
