TRICARE Prime
Use the TRICARE Prime Enrollment, Disenrollment and Primary Care Manager Change Form (DD Form 2876) to enroll in one of the TRICARE Prime options:
- TRICARE Prime
- TRICARE Prime Remote
- TRICARE Prime Overseas
- TRICARE Prime Remote Overseas
To enroll by mail, download, complete, and mail your region’s form to your regional contractor:

Other Enrollment Options
Enroll Online
- Go to the Beneficiary Web Enrollment portal.
- Click the red “Log On” link at the top of the page. You must have a Common Access Card, DFAS (MyPay) Account, or a DS Logon account to log in.
- Once logged in, select the “Medical” tab to enroll.
Call Your Regional Contractor
- East Region: 800-444-5445
- West Region: 888-TRIWEST (874-9378)
- Overseas Region: Call the Regional Call Center for your overseas area.
Downloading TRICARE Forms
To download an enrollment form, right-click and choose “Save Link As” or download directly from the WHS forms page. For enrollment in TRICARE Prime options, TRICARE Select options, or TRICARE Young Adult, use your region-specific DD enrollment form.
