Required Documents
Remember to bring all required documents to your appointment to update the Defense Enrollment Eligibility Reporting System(DEERS) A database of information on uniformed services members (sponsors), U.S.-sponsored foreign military, DoD and uniformed services civilians, other personnel as directed by the DoD, and their family members. You need to register in DEERS to get TRICARE. or receive your ID card. Call the ID card office first if you have any questions.
Each family member’s eligibility record must be updated separately. To register family members in DEERS, sponsors must:
- Complete a DD Form 1172.
- Provide documentation (see below).
If the sponsor is unavailable, family members can update DEERS. To do this, you must have a DD Form 1172 signed by your sponsor within 90 days or a valid power of attorney.

Attention TRICARE Users: myAuth will roll out in phases. This began with milConnect and ID Card Office Online on May 21. Over time, all DOD applications that use DS Logon will transition to myAuth.
