Enroll in DEERS

  • Defense Enrollment Eligibility Reporting System (DEERS) is a function of the military services’ personnel departments. They establish eligibility and update the DEERS system. Ref: 32CFR199.3 (a)

  • milConnect is a Web site provided by the DMDC that allows sponsors, spouses, and their children (18 years and older) to access information regarding their personal information, health care eligibility, personnel records, and other information from a centralized location. Each main menu option within this site launches a page of Questions and Answers. Always make sure you keep your military ID card current and update DEERS whenever you move or have a life changing event like marriage, divorce, death, adoption, etc.


Reserve Component Benefits/Enroll in TRS

  • Your Reserve Component (RC) unit or personnel office must validate your qualifications for TRS and they are responsible for entering your TRS eligibility into Defense Enrollment Eligibility Reporting System (DEERS). Once your RC unit or personnel office updates your record, you can download the TRS request (enrollment) form (from the Reserve Component Purchase TRICARE Application), complete it and mail the form to the contractor along with a premium payment for one month. If you cannot access the Reserve Component Purchased TRICARE application, please contact your unit personnel office to ensure DEERS is correct.

Participate in TRICARE Standard/Extra

  • TRICARE Standard does not require enrollment. It is the basic coverage and is automatic when the DEERS enrollment is completed/current. Remember you can also use TRICARE Extra (does not require enrollment either) and save money by simply using a network provider.

Enroll in TRICARE Prime

  • If you live near a military treatment facility (MTF), you may be required to enroll with them. If not or if the MTF grants permission for you to enroll with a civilian provider, you can select a Network Primary Care Manager (PCM) at http://hmd.humana-military.com/South/main.asp. Complete a TRICARE Prime Enrollment Form DD2876 and mail to the appropriate regional contractor for processing. Always call the provider first and ask if they are accepting new patients. Ref: 32CFR199.17 (a)


  • You may enroll in TRICARE Prime online through the TRICARE Website. Go to https://www.dmdc.osd.mil/appj/bwe/indexAction.do. This will take you to the enrollment website. If you are not on a military computer using a Common Access Card (CAC), the easiest method of "authentication" is to select DFAS Pin. This is the same User ID and Pin number you would use when you go to the MyPay website to view your Leave & Earning Statement.

Change my PCM

  • You can change your PCM by going to http://hmd.humana-military.com/South/main.asp and choosing a new network provider. Always call and make sure they are accepting new patients. Complete a TRICARE Prime Enrollment/PCM Change Form DD2876 and mail to the appropriate regional contractor for processing. Ref: TOM 6010.51-M Chap 6, Sec 1


  • You may also change your PCM online. Go to https://www.dmdc.osd.mil/appj/bwe/indexAction.do. This will take you to the enrollment website. If you are not on a military computer using a Common Access Card (CAC), the easiest method of "authentication" is to select DFAS Pin. This is the same User ID and Pin number you would use when you go to the MyPay website to view your Leave & Earning Statement.

Find a network provider

  • Go to http://hmd.humana-military.com/South/main.asp and select the the kind of provider; physician, hospital, ancillary provider, etc. you are looking for. When you select your provider “By Distance From Zip Code”, the system will give you a list of providers in your area. Make sure you always call the provider’s office to see if they are still accepting new patients. Ref: TOM 6010.51-M Chap 6, Sec 1

When you travel with TRICARE Prime

Transfer my TRICARE Prime enrollment from one Region to another

  • Moving Made Easy:

    • If you are an ADSM or ADFM moving to a new location, the easiest way to transfer your Prime enrollment is to call your current regional contractor to begin the process. If you are moving to a new region, your information will be sent to your new regional contractor, who will follow up with you to complete the enrollment transfer after you arrive at your new location.


  • After updating your address in DEERS, you can transfer your enrollment online via the Beneficiary Web Enrollment Website. If you are not on a military computer using a Common Access Card (CAC), the easiest method of "authentication" is to select DFAS Pin. This is the same User ID and Pin number you would use when you go to the MyPay website to view your Leave & Earning Statement.

File a claim

  • If you are using a TRICARE network provider, you don’t need to do anything but give them your sponsor’s social security number and your personal information. Providers may also copy your military ID card. They will bill the claims for you. If you are not using a network provider, ask them if they will file the claim to TRICARE. If they won’t, mail a completed TRICARE Claim Form DD2642, a fully itemized bill, and attach Other Health Insurance Explanation of Benefits (EOBs), if applicable, to:

    Remember, if you are filing the claims, you have up to one year from the date of service to file your claim.

Get a prescription claim filed

  • Mail a completed TRICARE Claim Form DD2642, medication receipts and if you have any other health insurance, you will also have to include a copy of their EOB showing payment or denial to:

  • You have one year from the date you received your medication to file for reimbursement with Express Scripts. Ref: 32CFR199.7

File a TRICARE for Life claim

  • You don’t need to do anything if using a Medicare provider. Medicare is your primary payer and they will send the bill directly to TRICARE. If you have other health insurance (OHI), after Medicare pays, you will need to provide a fully itemized bill, copies of the OHI Explanation of Benefits (EOB) and copies of Medicare EOBs to TRICARE before they will pay. Get Dental Information

  • If you are an active duty family member, reserve component, National Guard, and you have enrolled in the TRICARE Dental Program, the dental insurance company is MetLife. They can be reached at 1-855-638-8371 or at https://mybenefits.metlife.com/tricare. Ref: 32CRF199.13


  • If you are a retiree or retired family memthe dental insurance company is Delta Dental. They can be reached at 1-888-838-8737 or at www.trdp.org. Ref: 32CFR199.22ber, and have enrolled in the military’s dental plan,


  • If you are active duty, the Active Duty Dental Plan (ADDP) is administered by United Concordia (UCCI). They can be reached at 1-866-984-2337 or www.addp-ucci.com.
