Line of Duty Dental Care
You may qualify for Line of Duty (LOD) dental care when an injury or illness occurs during active duty status and requires dental treatment. You must have a LOD determination from your Service signed by your commander or designated representative. Send the LOD documentation to United Concordia in one of the following ways:
United Concordia Companies, Inc.
P.O. Box 69431
Harrisburg, PA 17106-9430
If your LOD determination is approved, you are only eligible for treatment for that injury or illness. United Concordia will tell you if you have been approved for LOD dental care and what type of treatment is authorized.
Submitting the LOD documentation is not an approval to get the dental care. You will be notified when your LOD determination has been approved. If you have a dental emergency and feel that you cannot wait for the Government review and authorization, you may be responsible for all charges associated with that care.
To learn more, contact United Concordia at 1-866-984-2337 or view the Line of Duty Dental Care Flyer. Family members do not qualify for LOD Dental Care.
